ConcertoCare is an in-home medical provider that gives you an extra layer of support through personalized, in-home medical services that will help manage your health. You can keep your current primary care provider, specialists, and all the benefits you already have through Humana.

Frequently Asked Questions

Humana has partnered with ConcertoCare as part of its commitment to improving its members’ health and well-being. The program is meant to provide Humana members with a new option for easy to access, in-home medical care and services that can help them manage their health.

Humana is initially offering the ConcertoCare program only to select eligible members. If you are eligible, you should have received a letter in the mail and may have also been contacted by phone. If you are a Humana member who did not receive any outreach, you can still contact us at 888-735-7544 (TTY: 711), 8 a.m. to 5 p.m., Monday through Friday to check your eligibility find out if you are eligible.

If you are eligible and decide to join the ConcertoCare program, contact us at 888-735-7544 (TTY: 711), 8 a.m. to 5 p.m. Monday through Friday. We take care of the rest in an easy three-step process:

  1. We schedule your first in-home visit. This will be based on your schedule and needs.
  2. Before your first in-home visit, we will call to confirm your appointment and ask some questions. This helps our team plan for the visit and provide the best care possible.
  3. During your first visit, you will be asked to sign ConcertoCare’s short program agreement. Then we conduct an initial check-up to help us understand your health and needs.

From there, our teams work with you and your regular doctor to develop a personal care plan that will help manage your health and well-being.

Examples from a care plan could include:

  • Traditional medical care such as a specialist follow-up or help with following a certain medication plan.
  • Fitness plans.
  • Coaching sessions.
  • Connections to community resources.

A care plan is personalized to your needs.

Depending on your needs, you may have access to a whole team dedicated to you. This includes regular in-home checkups, urgent visits, phone access to our team, and so much more.

ConcertoCare offers in-home care and services designed to meet each member’s individual goals and needs. There is no one size fits all.

  • It might mean support to manage one or more chronic conditions. This could help with your goal of reducing the number of flare-ups that cause hospital visits.
  • For someone else, it could mean an annual in-home checkup and a little bit of coaching. This could help with a goal for weight management or staying connected with family who lives far away.

If you are healthy, it is still advantageous to enroll in ConcertoCare now, because it is an investment in your future and your peace of mind. With just a short initial in-home visit, you will get to know a team who personally understands your health.

If there is a change to your health, the care team is on call to help you when you need them. You will be able to call a provider who already knows you and who is available. Someone will come to your home, and there are no bills to deal with after the visit.

There is no additional cost to you for joining the program, now or ever, as a part of your Humana plan.

ConcertoCare is a completely optional program that is offered as an “add-on” to everything you already have access to as a Humana member. Joining the program does not change your normal provider, and it does not change any of your other existing benefits or services.

If you decide to try the program and it isn’t right for you, that’s okay. Your plan will not change, and there will be no additional paperwork.


How Does ConcertoCare Help Providers?

ConcertoCare is an in-home provider of medical, pharmacy, and behavioral services. We have collaborated with Humana to help manage the care of a portion of its members. ConcertoCare does not replace the care that you deliver as a primary care provider or specialist. Instead, our services are intended to help you manage the health of your patients by augmenting the care you provide as part of your existing physician-to-patient relationships.

Our multidisciplinary teams work with you and your staff to:

  • Help jointly develop and manage care plans for your patients that encompass the full range of their medical, pharmacy, behavioral, and social needs.
  • Help your patients with day-to-day activities that allow them to manage their health in between their visits with you – things like making sure they understand and consistently take their medications, supporting them in self-managing their conditions and knowing when to seek additional care, helping them arrange transportation to medical visits, and more.
  • Help you stay on top of when your patients need your attention – for example, when they have a sudden change in health status or have an acute visit Help your patients connect with and make use of additional community resources that benefit them.  

What does collaborating with ConcertoCare look like?

ConcertoCare's teams are here to work with you in service of your patients. Knowing that every practice is unique, we are flexible in our approach to working with you and your team. We can communicate with your practice in whatever way is most seamless – including phone, fax, or EHR-based information sharing.

Pre-visit communications

Whenever a patient opts into the program and prior to our first visit with them, ConcertoCare will contact the patient’s primary care provider, inform them of our intent to engage with a patient in their practice, and request medical records for pre-visit preparation.

Post-visit communication

Immediately following all home visits, our team faxes documentation and treatment plans to patients' PCPs and as applicable, key specialists, and work to align on the patient's care plan. We also work to coordinate identified patient needs such as orders, referrals, or prescriptions.
