Website and Information Security. We maintain reasonable administrative, technical, and physical safeguards designed to protect the information that you provide on this website. However, no security system is impenetrable, and we cannot guarantee the security of our website, nor can we guarantee that the information you supply will not be intercepted while being transmitted to us over the Internet. We are not liable for the illegal acts of third parties such as criminal hackers.

Voice or Text Messages: If you have provided your mobile phone number and have agreed to receive voice and text messages, you may receive messages to that number that may include appointment reminders, care coordination and follow-up messages, and other messages related to your care and your clinical relationship with us. Your receipt of text messages is voluntary. You may still receive healthcare services even if you choose not to receive text messages.

There is no fee charged to receive messages. Message and data rates may apply– this can be checked with your mobile service provider. Charges are billed and payable to your mobile service provider or deducted from your prepaid account. Consent is not a requirement.

Alerts sent via text/SMS may not be delivered if the mobile phone is not in range of a transmission site, or if sufficient network capacity is not available at a particular time. Even within a coverage area, factors beyond the control of the wireless carrier may interfere with message delivery, including the customer's equipment, terrain, proximity to buildings, foliage, and weather. The wireless carrier does not guarantee that alerts will be delivered and will not be liable for delayed or undelivered messages. Supported carriers include AT&T®, Verizon® Wireless, T-Mobile ®, Metro PCS®, Sprint®, Boost Mobile ®, Virgin Mobile® , U.S. Cellular®, and others.  As a recipient of mobile alerts, you will receive up to two (2) automated messages prior to each appointment to the mobile number provided. You can unsubscribe from this service at any time by texting 'STOP' in response to a text message that you receive from us. Upon texting STOP, your phone number will be unsubscribed from this service, and you will not receive any further health and wellness messaging notifications via text.

By subscribing to alerts, you may receive, from time to time, further text messages from the practice and service providers of the practice. Message & data rates may apply.
