How Do I Join?

Our CarePartners program is delivered in partnership with health plans and primary care providers (PCPs). If you live in California and are eligible to enroll with us, your health plan will reach out to you directly.

If you have recently been contacted by your health plan about ConcertoCare’s program and are looking for additional information, please call us at the phone number listed on your outreach materials to speak to one of our new member specialists.

What We Do

What We Do

ConcertoCare's CarePartners program offers a full array of personalized in-home medical care and other supports designed to keep you healthy and live life on your terms. We work with your existing PCP and coordinate our services in concert with them -- this includes specialist providers too.

Here's what you can expect when you first enroll in our program:

  1. We confirm your interest in the program – we coordinate directly with your health plan, so there are no forms to fill out
  2. We schedule your first in-home visit 
  3. Our provider visits you at your home and conducts a full evaluation of your health needs and personal goals
  4. We develop a care plan that is customized to you 
  5. You are now a ConcertoCare member – we regularly help you manage your healthcare needs throughout the year, and you have access to the full set of our services if something comes up
